Definitions
- Assessment: Treatment request evaluation conducted by an Authorized Healthcare Professional through the Felix Platform
- Authorized Healthcare Professional: Collectively “Authorized Healthcare Professionals”, Physicians, Nurse Practitioners, and Pharmacists licensed to practice in a province or territory of Canada
- Healthcare Services: Collectively, Assessment review by Authorized Healthcare Professionals, Pharmacist Services, treatment prescribing, and treatment dispensing
- Felix Health Platform: Collectively, the hardware, software, applications, websites, content, products and services owned and/or operated by us to enable the provision of remote healthcare services to Users, among other purposes
- Felix Pharmacy Network: Collectively, the Ontario and British Columbia Felix Pharmacy locations
- Patient Portal: Your Felix account within the Felix Health Platform, accessed using unique login credentials assigned to you
- Personal Information: Collectively “Personal Information” (information about an identifiable User as defined by the Applicable Law) and “Personal Health Information” or “PHI” (information about a User’s health or healthcare as defined in the Applicable Law)
- Treatment and Service Recommendations (Recommendations): Prescription and/or non-prescription health products recommended to a User in their Patient Portal, based on their medical history
- User: (Hereinafter “you”, “your”) An individual registered to use the Felix Health Platform
Consent
I acknowledge that I have read and fully understand the risks, limitations, and scope of receiving Recommendations through my Patient Portal in the Felix Platform. I understand and accept the risks outlined in this notice associated with Recommendations. I consent to the scope and limitations outlined in this notice.I acknowledge that I may, at any time, withdraw my consent to receive Recommendations. Any questions I had have been answered. If I have any further questions or concerns regarding the handling or storage of my information in reference to Recommendations, I understand I can review Felix’s Privacy Policy and/or contact the Felix Health Privacy Officer, Emma Stern at privacy@felixforyou.ca.
Scope of Recommendations
The purpose of Recommendations is to suggest treatment(s) that may enhance the care you are receiving within and/or outside of the Felix Platform, based on your medical history. No results or outcomes can be guaranteed should you proceed with a Recommendation. Recommendations may include Healthcare Services, Pharmacy Services, prescription, and non-prescription health products, available through Felix Health and the Felix Pharmacy Network. Recommendations are separate from Healthcare Services you are currently receiving, or have received in the past, through the Felix Platform. Recommendations should never be interpreted as medical advice. In order to receive a prescription health product, an Assessment and subsequent approval by an Authorized Healthcare Professional is required. You should always consult an Authorized Healthcare Professional within or outside of the Felix Platform prior to the use of any non-prescription health products. Prescriptions and treatment plans are issued at the discretion of the assessing Authorized Healthcare Professional.Recommendations are generated based on the below outlined Personal Information, provided by you in your Felix Patient Portal. Access to Recommendations may vary by province and product availability.
Collection, Use, and Disclosure of Personal Information
Recommendations are generated within the Felix Platform using your Personal Information, pursuant to your consent. Personal Information that may be used to make Recommendations will vary depending on the circumstances, but may include:
- Sex at birth, gender
- Age
- Allergies
- Medical conditions
- Medical history (eg. medications, past hospitalizations)
- Height
- Weight
- Health products (eg. medications, supplements)
- Lifestyle factors (eg. smoking habits, exercise habits, stress levels)
The Personal Information used to generate Recommendations are only available in your Patient Portal. Recommendations will be made available to you through your Patient Portal, or via electronic communications, as applicable. Please refer to our Privacy Policy for full details on how Personal Information is collected, used, and disclosed at Felix.
Potential Risks of Recommendations
Felix takes reasonable precautions to reduce the risk associated with the transmission of Recommendations, however they cannot be eliminated entirely. Risks receiving Recommendations may include (non-exhaustive):
- Recommendations may trigger uncomfortable emotions depending on the experiences you have had with care and your health history
- Recommendations may conflict with personal beliefs or values
- Recommendations may not be suitable for you. Consultation with an Authorized Healthcare Professional through or outside of the Felix Platform may be required
- Recommendations may pose additional privacy risks in the event of unauthorized Patient Portal access. Strong account protection as outlined in the Terms of Use is strongly recommended
You may also revoke your consent to receive Recommendations at any time, through the Profile section in your Patient Portal.
