Please read this page before booking. It sets out what Felix is providing, what Felix Health Inc. (“Felix”) is not providing, and what information will be disclosed in order to arrange your appointment.
If you experience symptoms that concern you, or are experiencing a medical emergency, attend your nearest emergency department or call 911. Do not wait on an elective test result to address an immediate health concern.
Definitions
- Assessment: Treatment request evaluation conducted by an Authorized Healthcare Professional through the Felix Platform
- Applicable Law: Laws and regulations Felix, Authorized Healthcare Professionals, and/or Users are subject to
- Authorized Healthcare Professionals: Collectively, physicians and nurse practitioners licensed to practice in a province or territory in Canada providing care through the Felix Platform
- Felix Platform: Collectively, the hardware, software, applications, websites, content, products, and services owned and/or operated by us to enable the provision of healthcare services to Users, among other purposes
- Medical Record: Any User information (collectively, Personal Health Information and Personal Identifying Information) that is used for the facilitation of Assessment and healthcare services
- Personal Health Information (PHI): Identifying information about an individual’s health including, but not limited to, health history, health care, and eligibility to receive insurance coverage for healthcare services
- Personal Identifying Information (PII): Identifying information; this may include, but is not limited to your name, address, and phone number
- Referral: The process by which Felix, at your request and/or on the suggestion of an Authorized Healthcare Professional, arranges an appointment or connects you with a Third-Party Provider for a service that Felix does not provide directly, and discloses the PHI and PII described in this notice to that provider for the purpose of arranging and delivering that service.
- Third-Party Provider: An independent external clinic, testing facility, or healthcare provider that is not owned, operated, employed, supervised, or controlled by Felix, and to which you may be referred to receive a service that Felix does not provide directly (for example, a sleep clinic, or a VO2 max or DEXA testing facility).
- User: (as follows, “you”, “your”) an individual registered to use the Felix Platform
Overview
From time to time, you may be offered a Referral to a Third-Party Provider for a service that Felix does not offer directly. This notice outlines what information we share to make a Referral, why we share it, who receives it, and the choices you have.
Disclosure of your information to a Third-Party Provider is only done with your express consent. This is collected when you accept the consent statement shown at the time a Referral is offered (e.g. within an Assessment). All Referrals are optional and not a condition of using Felix services unless otherwise stated. The type of Referral affects how “elective” it is: Longevity Add-On tests (including VO2 max and DEXA) are entirely elective and arranged at your request. Referrals offered through the Obstructive Sleep Apnea (OSA) category of care are also optional, but are suggested where appropriate following clinical assessment.
Third Parties
All Third-Party Provider clinics are independent third parties.
Felix does not employ, supervise, direct or control these clinics or their personnel, and does not verify their equipment, accreditation, protocols or the accuracy of their results. Felix is not responsible for a clinic’s acts or omissions, for the conduct of the test, or for the accuracy, completeness or timeliness of any result produced by the clinic.
Whether a Felix Authorized Healthcare Professional interprets your results depends on the test. Longevity Add-On Tests are the only Referral where no Felix Authorized Healthcare Professional recommends the test or provides clinical interpretation of your results. The specific terms of these tests are outlined in the “Longevity Add-On Tests” section below.
General Privacy Terms
Available Providers
We work with a network of clinics and testing providers (“Third-Party Provider”). The specific Third-Party Provider you are referred to may depend on factors including your geographic location, the service(s) being requested, and your clinical needs, depending on the nature of the Referral. As a result, the specific Third-Party Provider to whom you are being referred may not be known at the time you provide consent. The Third-Party Provider we work with for each type of service are listed in the sections below. Disclosure of Information
Information is disclosed so that the receiving Third-Party Provider can determine appropriateness of providing services, contact you to schedule an appointment, and deliver the requested services.
The specific information being disclosed depends on the services being requested, as outlined below.
Referrals are transmitted by fax, and only include PHI and PII that are reasonably necessary. Once your information is received by a Third-Party Provider, it becomes subject to that provider’s own privacy policies, as well as Felix’s. For more information about an external provider’s privacy practices, please reach out to them directly.
Withdrawing Consent
Sharing your information with a Third-Party Provider only happens after express consent has been provided. You may withdraw your consent at any time by contacting [email protected].
Withdrawing your consent will stop any further disclosure to external providers, however it may not be possible to retrieve information that has already been shared. In the case where disclosure has already occurred, you will need to contact the relevant Third-Party Provider directly regarding information about you that is in their possession.
Retention
Felix retains a copy of your Referral and your consent for as long as necessary to administer the Referral, meet our legal and regulatory obligations, and maintain your health record. After this time, it is securely disposed of in accordance with our destruction practices.
Longevity Add-On Tests
Includes:
- VO2 Max testing, and
- DEXA scans
The clinic performing the scan is responsible for your care during the appointment, including its own screening, consent and safety procedures. Questions about how the test is performed, how to prepare, or whether it is suitable for you should be directed to the clinic.
Referral Limitations
- Your results will be provided to you and, where applicable, to the clinic that performed the test. No Felix practitioner will review them.
- Felix cannot interpret your results, advise whether they fall within a normal range, answer questions about their meaning, or make any treatment decision based on them.
- Results will not be sent to Felix for clinical review.
- Questions about your results should be directed to the clinic that performed the scan, or to a family physician, nurse practitioner or other in-person clinician.
- You accept responsibility for deciding whether to attend, and for disclosing relevant health information to the clinic.
Disclosure of Information
To arrange your appointment, Felix will disclose the following to the Third-Party Provider for the purposes of booking your test:

This information is disclosed solely to book and confirm your appointment. Felix does not disclose your medical history, prescriptions, consultation notes, or any other content from your Felix record without your consent.
You may withdraw this specific consent at any point before the booking request is sent, in which case Felix will be unable to arrange the test. Information already disclosed to a Third-Party Provider cannot be reversed.
Findings that Require In-Person Care
Third-Party Provider tests may identify certain abnormalities or findings that cannot be assessed or managed virtually:
- A VO2 Max test may identify cardiac abnormalities, pulmonary and ventilatory abnormalities, and low cardiorespiratory fitness as a mortality risk marker.
- A DEXA scan may identify low bone density, osteoporosis, or an elevated risk of fracture.
Appropriate follow-up ordinarily requires care that Felix is not able to provide via virtual means, including physical examination, in-person assessment of fracture and fall risk, bloodwork, further imaging, and monitoring over time, often coordinated alongside other aspects of your health.
If your results indicate an imminent health concern, you will need to seek out care with an in-person clinician – ideally a family physician or nurse practitioner – who can follow you longitudinally and address the underlying health risk or condition identified through these tests. If you do not currently have a family physician or nurse practitioner, resources to help you find one are available here.
Acknowledgement
By consenting to receive a Longevity Add-On test, Felix is arranging a test for you at an independent clinic that is not owned or operated by Felix.
For these services:
- Felix is acting solely as a booking facilitator and is not providing, supervising, ordering, recommending, or reviewing any part of it
- No Felix Authorized Healthcare Professionals will review or interpret your results, and you will not rely on Felix to do so
- Booking a scan through Felix does not establish a patient relationship with a Felix practitioner with respect to this test, and does not alter or extend any care you may already receive through Felix.
- Results indicating a health risk (e.g. osteoporosis) requires in-person care, and arranging that care is your responsibility
Sleep Clinic Referrals
If your responses to our Obstructive Sleep Apnea (OSA) assessment indicate that you may benefit from a professional sleep assessment, Felix may offer to refer you to one of our sleep partner clinics. We will only make this Referral and disclose the required information with your express consent, which is provided by accepting the consent statement shown in the OSA intake quiz.
Sleep Clinic Partners
Felix works with a network of sleep clinic partners. The specific clinic you are referred to depends on factors such as your geographic location and your clinical needs. Because the appropriate clinic is determined based on these factors, the specific clinic may not be known at the time you provide your consent. Your Referral may be sent to one of the following partner clinics:
- MedSleep (primarily serving patients in Ontario, British Columbia, and Alberta)
- Other external sleep clinics that will be identified on a case-by-case basis
You will be notified of the sleep clinic your Referral has been sent to by the receiving clinic to coordinate booking details.
Disclosure of Information
Information is disclosed so that the receiving clinic can determine appropriateness of providing services, contact you to schedule an appointment, and provide you with a sleep assessment and any resulting treatment.
By consenting to be referred to an external sleep clinic, the following categories of personal information (including personal health and identifying information) will be disclosed to the receiving partner clinic:

We only share information that is reasonably necessary to support the Referral and the sleep clinic’s assessment.
Fees
Any additional costs associated with your OSA Referral (including test fees, treatment, equipment or CPAP machines, and any other services provided by the Third-Party Provider) may be extra and are not covered by Felix. These costs will be billed to you directly by the Third-Party Provider you are referred to.
Acknowledgement and Understanding
By consenting to a Referral, you acknowledge and understand the following:
- You consent to Felix disclosing the information listed above to the clinic for the described purposes
- You understand that the receiving Third-Party Provider is an independent provider that Felix does not own, operate, employ, or control
- You understand that Felix cannot guarantee that any test included in a Referral is medically appropriate or safe for you, and that Felix may not screen you for all possible contraindications
- You accept responsibility for deciding whether to attend, and for disclosing relevant health PHI and PII to the clinic other than what is listed
- You understand that results indicating certain health conditions require in-person care and that you are responsible for arranging this care
- You acknowledge and agree to the additional terms applicable to the specific test you are requesting, as set out in the relevant sections above (including, for Longevity Add-On tests, that no Felix practitioner will review or interpret your results)
- You are the age of majority in your province of residence and have the capacity to provide consent.
